Terms & Disclosures
These terms govern your use of myguardiance.com. By using the website or submitting a request, you agree to these terms.
1. Insurance information only
This website provides general information and a way to request contact from an insurance advisor. It is not an insurance application, quote, binder, policy, eligibility determination, or guarantee of coverage. Submitting the form does not enroll you in a health plan and does not create an agent-client, fiduciary, or insurer-insured relationship.
2. Marketplace and government disclosure
Guardiance Insurance Group is a privately owned insurance agency. It is not affiliated with or endorsed by the U.S. government, HealthCare.gov, the Centers for Medicare & Medicaid Services, or any state or federal Health Insurance Marketplace. Official Marketplace information is available at HealthCare.gov or by calling the Marketplace directly.
3. $0 premium disclosure
$0 monthly premium plans may be available to individuals who qualify for advance premium tax credits, depending on household income, household size, location, age, tax-filing status, immigration status, plan availability, and other eligibility factors. Not everyone qualifies. A $0 premium does not mean health care is free; deductibles, copayments, coinsurance, out-of-network charges, and non-covered services may apply.
4. Accuracy and plan availability
You are responsible for providing accurate information. Coverage, premiums, subsidies, provider networks, drug formularies, benefits, and plan availability can change and must be confirmed using official plan documents and Marketplace eligibility results. Guardiance does not guarantee that every carrier or plan available in your area will be offered or discussed.
5. Communications consent
When you check the consent box and submit the form, you provide express written consent for Guardiance Insurance Group to contact you at the phone number and email address you supplied through calls, text messages, and emails about health insurance options, including communications made using automated technology. Consent is not a condition of purchasing goods or services. Message and data rates may apply. Message frequency varies. Reply STOP to opt out of marketing texts and HELP for help. You may also request that calls or emails stop.
6. Acceptable use
You may use this website only for lawful personal purposes. You may not attempt to disrupt the site, submit false or misleading information, impersonate another person, scrape or resell site content, introduce malicious code, or use the website in a manner that violates applicable law.
7. Intellectual property
The Guardiance name, logo, website design, copy, and related materials are owned by Guardiance Insurance Group or used with permission and are protected by applicable intellectual-property laws. No rights are granted except the limited right to use the website for its intended purpose.
8. Disclaimer and limitation of liability
The website is provided “as is” and “as available.” To the fullest extent allowed by law, Guardiance disclaims implied warranties regarding the website. Guardiance is not liable for indirect, incidental, special, consequential, or punitive damages arising from use of or inability to use the website. These limitations do not apply where prohibited by law.
9. Privacy and changes
Our Privacy Policy explains how we process information. We may update these terms by posting a revised version and changing the effective date. Continued use after an update means you accept the revised terms.
10. Contact
